So personally I seem to still be killing it, I have no complaints about my endurance, explosive strength, reaction times, muscle recovery anything else except my ability to flush inflammation, and also am noticing my knees just don't have as much cartilage as they used to. Not overall, but just specifically very much at one point on the inside where the VMO meets the knee and just below. I do have issues with kneecap tracking, they're not all over the place, just a bit to the outside when I bend my knees and always have been. I've been using muscle stim on my vmos for the last decade since I tore an acl.
Where should I go in the Western US for stem cells? Anything else I should do? I keep hearing hydrogel implants are going to be the tits (young knees rather) but never hear about them coming to market. I do feel like most of my knees are fine and its just a very specific point, so the hydrogel sounds so appealing. Thots? (Do thots keep you young?)

Lots of really great science on how important everything (mental health, sleep, mobility) is in addition to the nutrition and exercise we all know about. Hitting 40 this year and in addition to my 2 resistance days in the home gym I'll be adding 1 day of rucking in the hills with a weighted vest. Going to work more V02 max intervals into my mountain bike rides. I'm also going to go way beyond the recommended wrt cancer screening. Also got my genes tested to see what things I may be susceptible to as I age.

It's really easy to eat 2000 calories a day. It's hard to burn 2000 cal/d iif you have a sedentary job. A big part of obesity is how many calories you're burning when you aren't exercising and whether your caloric intake is stored as fat or kept handy to burn. People who have bad injuries or big operations typically continue to lose weight for weeks after they start eating well, then start saving calories and wind up 10-15 pounds over their original weight. I had a guy with a 80% burn--crashed his plane, killed his wife poor guy*. He was eating 8000 cal/d of Ensure and losing weight--lying in bed 24 hr/d. It's complicated.
*He also thought his hands in splints were sharks.
Last I checked stem cell treatments are around $10k each. There are a lot of positive anecdotes but also lots of ineffective ones, and there are no studies or methods that can say whether they're going to work for you, or how. Seems like a really expensive gamble.
If we're going to just generally speculate on the causes of the obesity epidemic:
-Obesity has risen proportional to total calorie consumption, which has risen by about 500 kcal/day in the last 50 years.
-60% of food eaten now qualifies as "ultra processed," up from something like 10% 50 years ago.
-People fed highly processed diets ad libitum will spontaneously eat about 500 kcal/day more than people eating a minimally processed diet ad libitum.
On average, that's most of the answer. Throw in some endocrine disruption from phthalates and epigenetic factors that make the children of obese parents more prone to obesity themselves and you can cover most of rest. True genetic obesity like leptin receptor mutations, etc. excluded, obviously.

On a more serious note: I find it much harder to change my eating habits than exercising more. ( if I have time).

Certainly eating more calories has a lot to do with the increased rates of obesity, but my point is that there are thin people who eat and exercise the same amount as fat people and the difference has to do with metabolic factors and baseline activity levels. As an example I've been thin most of my life despite eating a lot and not exercising formerly as much as I should--one reason being I can't sit still. At work I paced constantly between patients. Unfortunately, in recent years a lot of my nervous energy has been expended on this forum. It's TGR's fault I need to lose 15 pounds.
The whole processed sugar think for me fall into the "just because it is simple, doesn't mean it's easy" category. For example, my wife working in an office and the amount of candy bowls, potlucks, birthday cake, donuts and such is insane. Hell, apparently once a month the vending machine guy just gives away the out of date product.I don't buy sweets anymore because I eat them if they're there. Just like drugs

I'll try and find it later but I was watching a documentary that basically supported thinking about sugar as a recreational drug. Not having it the house is huge for me.
The kids thing cuts both ways. I know a lot of people that feed their kids like shit and they eat there food. I think how you eat growing up really sets your trajectory in life. Those kids throwing a tantum for a soda at the supermarket, they are sugar addicts. But Mom gives in, because she wants a pop too.
Diet soda might be better, but it's not good. You get to skip the calories, but it simulates the same responses in your body.
The recent New Yorker piece on Taco Bell quotes a former CEO’s book where they stated a theory that the change from from manual to auto trannies would result in a rise in obesity in the US because people would be able to put a snack in their right hand.
the last person I met who told me " The BMI doesnt work for me " wasn't fat either, but he was literaly about 100 lbs overweight
don't forget there is a white label on the side of every food product telling you what amount the portion size is and the sugar for that portion which you can compare to every other brand of the same product on the shelf
white label even tells you how many potato chips in a serving and its not the whole bag

I used to compete in competitive jiu jitsu, and I would cut down to 190 from my in-shape weight of 205. I had a six pack, did little to no weight training that would add loads of muscle mass, and was still considered “overweight” by my BMI. Even after I cut fifteen pounds of water weight I would be at 25.2 BMI which is over the normal BMI range.
Nowadays I’m rightfully categorized as overweight, but I’d have to be pretty sickly to ever be “normal.”
well ya the BMI does not do all that well with muscle either too much or too little but for the average MD in narth America it is a useful tool for tens of millions of sedentary overeating patients they can use to say " it would be good for your health if you lost weight " and like i said this guy was 100lbs overweight

It doesn't seem like it should be much more difficult from as anesthesiologist's point of view. They just need to up the dose to knock the patient out, right? Of course I suppose the fact they're obese probably increases the chance for something to go wrong.

buddy told me normaly he didnt even wear a mask precovid just sat behind a divider in the OR monitoring vital signs and reading a copy of the Guardian to stay awake, but I've heard he was really good
i think there might be a bit of an art to putting the patient under without killing them or having them wake up
But i think ultimately you really don't wana be on that table in the 1st place which starts with good health/ proper weight

Id imagine how an obese patient metabolizes the drugs (ie enough to keep them under, but not so much that they are groggy and shitty for days afterward due to storage in their fat), along with all the conditions they have which are likely to cause them to stop breathing when sedated. Basically, i would imagine there is a more narrow tolerance for dosages/specific drugs when the patient is obese as opposed to when the patient is fit and healthy.
Right. Dont put 20 gallons into an excursion and expect to be able to drive the same distance as a prius will go on 20 gallons. Know what car you are currently driving. How efficiently you burn/store calories is constantly changing (calories out) and can be difficult to control so you should focus on the thing thats easy to control- how many calories you are putting into your system. It ALL comes down to calories in vs calories out.
good stuff all. i've been on again/off again w my strength training the last few years coinciding w some chronic shoulder issues. i'm back on again but gonna try and go w 2 full body workouts/week. this is coupled w 2-3 days of skiing/biking/trail running and some stretching/yoga in the evenings. anyone want to critique this routine? i'm mainly just looking to "maintain" and keep some weights in my life. i think my main question is if there's enough volume in here. (late 40's using a home gym)
A:
5-10 min warm up
barbell back squat - warm up set then 4 sets of heavier weight 6-8 reps.
barbell bench -same as above.
rev grip barbell rows - same as above
about 6-8 band exercises focusing on my shoulders-this seemed to help w my initial shoulder issues and have kept it in the routine.
dumbbell stiff leg deadlift - 10-12 reps for 2 sets
5-10 min cool down/stretch
B:
warm up
trap bar deadlift - warm up plus 3 sets 6-8 w heavy weight.
dumbbell floor press - warm up plus 3 sets of 8-12. feels better for shoulders than normal dumbbell bench.
dumbbell supported row - 3 sets of 8-12 reps
arnold press - 3 sets 8-12 reps.
dumbbell lunge - 2 sets 8-12 reps.
cool down
2 minutes rest in between heavy weights and 60-90 sec for the higher rep stuff. i've done 3 and 4 day workouts in the past and would rather gain some extra time for outdoor rec and/or yoga/stretching so...

You guys are making me prioritize the weight setup I’ve been procrastinating on and know I need. I will likely lift only once a week and not sure heavy. What should I buy for a rack bench and bar? I’m a cheap bastard but want ship that won’t break.
