Maybe I don't or maybe there's not a consense-less consensus. The really violent ones, snuff porn, rape scenes etc ...meh. Not so 'fucked up' in my terms, just lowbrow.
Waters, Greenaway, Bunuel, Lynch..those guys make moves that have that twist which qualifies in my view.

I remember seeing The Cook The Thief His Wife & Her Lover in college with my hardcore vegetarian friend.

We were both a little zooted, which made Jean-Paul Gaultier's costume design and Ben van Os/Jan Roelfs' production design amazing. But my friend almost blew chunks during the "meat wagon" sequence.

Still, that film was so artistically done that it overshadowed the "fucked-upedness."

I'm trying to think of films that have made me squirm and have made me uncomfortable.

The only ones I can think of off-hand are:

Last House on The Left...early Wes Craven exploitation film that is just creepy and has one scene that'll make you gag (I remember renting the movie after reading a review of it by Roger Ebert in which he said it was vile, base, and exploitive, but it was also unnerving and scary and because it accomplished what it set out to do he was recommending it as a bona fide good horror film).


Crash...the whole Rosanna Arquette character freaked me out, especially the sequence where she goes shopping for a new car.

Come to think of it a lot of the early Cronenberg flicks were f'ed-up (Rabid, anybody?)

Irreversible was gruesome, but as with Greenaway, the direction was so arty that it overshadowed the unnerving bits. It was still intense.

Some of Paul Verhoeven's Dutch work (Spetters and Turkish Delight come to mind) can be a little unnerving, at least compared to most American fare.

I also think of films that leave you stunned as being f'ed-up. La Haine springs to mind on that front.

That's the movie that lost Cannes festival Palme d'Or to Moore's 911... It got the Grand Prix instead.

I'm not sure what fucked up really means (and if that's a good thing for a film). But I would guess that Michael Haneke shot quite a few contenders for the "most fucked up movie" I ever saw.

With your terms, I would second Waters and add Bill Plympton's I married a strange person to the list.

FUNNY GAMES - Unwanted Strangers Scene

Clockwork Orange-ish. In a way. Can a movie about violence ever rise above it's own violence? I'll have to watch it again, but so far as I recall there is almost no violence on-screen. It's the climate of violence that is so powerful. While watching this scene in Funny Games I remember thinking, "Absolutely nothing has happened so far but the is just unbearable!" Might be a hard movie to watch twice. Hmm.

I showed the scene (the YouTube link to Funny Games in my post above) to a female friend and after about a minute she was getting angry, "No, no, no. I don't want to see this. This is bad, this is going to be bad - I. Don't. Want. To. See. This. Turnitoff!"
And then I had to calm her down.
Don't know what would have happened if she'd seen the whole movie.
You sure feel impotent sitting there watching when you want to tear Pitt's head off...and I guess that was part of the director's point.
And the other guy, too, forget his name. Excellent acting, though. Doesn't feel like acting while you're watching, though.
That last scene in the boat, their indifference - even after everything else that has gone on it is still shocking.

"Happiness" is one of the most disturbing movies I have ever seen. It reminded me of a less surreal "Requiem for a Dream". This movie just made me feel uncomfortable, even after it was over.
