Interesting how the GI symptoms and the psychiatric symptoms have coincided. I think some of the discussed potential solutions have some validity. As icemang prolly knows, our gut microbiome has remarkable connections with our serotonin feedback circuits, with a direct line to our mental health. Too early to have solid data on this, but yeah, FMT is a consideration, although getting the "right" donor is completely undetermined in terms of suitability. Psychedelics are another interesting consideration based on what we're learning about neurobiology "rewiring" but again, we don't yet know how to do it "correctly" with insufficient clinical trial data.

Fuck, really tough position. And yes appears all mainstream resources have been exhausted. I echo the caution posted above specifically to using psychedelics to “rewire” ones neurochemistry — while we often like to highlight their success stories, there are many which do not end well. Speaking from experience a past roommate of mine ended his life early after one of these guided journeys went awry. I’ll check with several of the psychologist I work with for inpatient options on the west coast.

My sister dicked around with Ketamine Infusion therapy for her Depression.
Neither my brother in law or myself felt comfortable with the results. She abandoned the effort after a few sessions.

For research behind psychedelics I’d point you to Stanford as they seem to have the greatest body of work. They also may be able to refer her to somewhere if that is the route she chooses.

This is a tough thread to read, since I’ve unfortunately had several close relatives take their own lives. Wish there were an easy answer, but there isn’t one. Vibes, and I hope for the best.

This is going to sound simplistic in light of the other replies. But sunshine straight in the face (or artificial light in the PNW perhaps) and exercise is free and easy. Not saying it’s going to turn them around 100% but certainly won’t hurt anything. Best wishes.

I assume your friend has been only seeing doctors at Columbia if they still live in the area? Great hospital, but always worth a second or 3rd opinion from a different hospital.
As some of you know I have gone down this rabbit hole in the past with major depression problems which I didn't help by drinking like a fish. Kicked the drinking and drug stuff 9 years ago but then developed some really weird GI issues. For two years a highly acclaimed GI doc in the Princeton, NJ area told me I had Chrons Disease. I was taking chemo, tried every diet possible, was then giving myself humira injections, etc. Terrible time mentally and physically.
FINALLY I said fuck this and went to get a second opinion at Penn in Philly. They had it figured out in like 1 hour. Literally. Chronic appendicitis. Cut that bad boy out and I was good as new in like 48 hours.
Might not be so simple for your friend, but goes to show how important it is to get an unbiased second look.
I do assume she has really STRICTLY tried some major diet changes like gluten free, Low FODMAP, etc. You have to be 100% with those diets. My stomach issues are gone now, but I am super strict on no gluten. And I never tested positive for Celiac, but gluten really fucks with my head more than anything. I am dead serious. I eat some gluten and in 45 mins to an hour and I get low and anxious. It's crazy.
I have a friend who travels to asia alot. He got some kind of parasite that destroyed him for like 2 years as well. The saving grace was a diet specifically formulated to his gut that we was 1000% strict on for about a year I think. It saved his life. He is totally back to normal now, but he really had to stay strong and cook at home etc when his friends were going out to eat etc.
I know some type-A super successful people who get sick and don't want to listen to other doctors and end up fucking dying because they are so fucking stubborn and "always right".

Shittons of pre- and probiotic and, if she can, daily walks.
I have a gluten allergy that causes diverticulosis flare ups that have put me on the brink of a colon section many times and almost did a fecal transplant last year. Doc totally recommended it. Recently found gluten specific digestive enzymes and they have neutralized my gut issues immensely. I get pissed when I get gut issues but not depressed. But, you know what they say - the gut is our other brain. I would recommend a book called Death Begins in the Colon.

No addiction, was a super sharp girl, mother, PhD Chemistry/MatSci professor, silicon engineer.
She's done a ton of probiotic stuff, other GI drugs as well.
Is now constantly shaking, wracked with crying, insomnia, been going on for a couple of months.
She seems to have given up on even trying anything anymore.

It is. My wife had chronic hives for months and finally read something saying it could be a gluten intolerance. She was skeptical, but figured it was worth a shot. Stopped eating gluten for like a day or two and the hives went away. She hasn't had any gluten since. I think it's been almost a decade.

Dysbiosis, ie disruption of the Microbiota-gut-brain axis, has been repeatedly linked over the last 15 yrs with a number of neurological disorders, including a causitive agent for depression. Basically in animal models fecal transplants can instill depressive behaviors in "healthy" individuals, and revert depressive behaviors when a "healthy" microbiota is transplanted into a depressive animal. As much as I've looked into it, probiotics/diet/lifestyle changes are often not enough to bring about the full scale bacterial remodeling necessary to alter the microbiota makeup significantly. mostly due to delivery through the stomach being too harsh. As nasty as they sound, fecal transplants have worked for many for directly remodeling the intestinal flora.

While all the accolades are great, they are probably a hinderance to her getting the help she really needs. Some of the most intelligent people in the world are the most difficult for mental health professionals to deal with because the patients are self-reliant to a fault. Her stomach issues may not resolve until her mental health improves. Sounds like she is caught in quite a bad loop. Bad gut leading to mental issues which then exacerbate the stomach issues.

Maybe the initial bad food encounter was secondary to something already going on.
Is / was she running a research program, or involved with laboratory research that used toxic materials? Any chance that she was poisoned? I would imagine that blood work would have been done early on, but did those tests screen for either renal or hepatic derangement?Daniel Ortega eats here.

I know someone who had serious GI issues for years who had their life changed for the better after three days of back to back kambo ceremonies.
By all accounts kambo is very intense and is nothing to mess with. That said, it fixed this person's GI issues after all avenues in Western medicine had been pursued.

Last update — just heard back from a couple contacts. Try reaching out to Dana Free she comes highly regarded and is treatment placement specialist. After an initial intake she can point you to resources based on need. Again I have not personally interacted with her— but hear great things.

Well, the FDA hasn't given psilocybin- and MDMA-assisted therapy the "Breakthrough Therapy" designation for treatment of severe depression and PTSD because there isn't evidence of their effectiveness.

Sounds like ketamine would be a great option for your friend. They prescribe it in a nasal squirter thing. You can also go full k-hole in a controlled setting if your shrink is down with it. I've heard it works wonders on panic attack level anxiety
