Need rec for a Co in salt lake to make a yurt roof panel

I need to have a large insulated, waterproof, square with steel eylets at each corner made. I'm thinking large enough to cover a 5ft diameter circle. Each corner needs to have an eylet for tie down. Basically a big insulated tarp. Who in salt lake could make me one? I was thinking like Jack's plastic welding style Paco pads, but in a big square.

I know I've mentioned them on here before, but it's been a while. AAATopShop in Ogden seems to do quality work for a fair price. They do a lot of vintage auto and marine upholstery. The family who owns it are decent people.
