He also threw the package of the unknown substance overboard. It was not recovered.

Kenneth Swindal signaled for help, and a Fish and Wildlife Conservation Commission boat responded and took the two brothers to shore where paramedics met them.

Thomas Swindal was taken to South Miami Hospital where he died, police said.

An autopsy will be conducted to determine the cause of death, authorities said.

WEST SPRINGFIELD – Investigators have determined that the explosion that took the life of 33-year-old Daniel Martinez at Balise Honda Wednesday morning occurred when he used a disposable lighter to peer inside a 55-gallon drum of flammable cleaning solvent.

The state Fire Marshal’s office released its finding Thursday afternoon.

Martinez, who lived in a house close to the rear of the facility, was employed by Real Clean Automotive Detailing, a subcontractor that performs reconditioning work at the dealership.

Holy shit!

Not my girls in the photo but my neighbor(s). We were in maui with 2 other couples and all the kids. My wife and my 2 boys were not in that photo as we were behind the camera man (my friend who was taking the picture of the girls).

Crazy situation to witness. There's always a few guys that push the envelope and this guy paid the ultimate price. No respect for mother nature.

Short version - he had his back to the sea and didn't see the set wave coming. Hit the rocks and threw a wall of water up (visible in the photo) that took him out when it hit him. Knocked him down and pushed him towards the blowhole. All that water had to go somewhere and it went down the blowhole, taking him with it. It was like someone flushed a big toilet.

Horrific screams from all and his bro-in-law (the other guy in the photo) was panicing. We had to tell him to get away from the hole. Sent the family back up the trail to the cars and spent 20 minutes trying to figure out if there was anything we could do. Climbed the cliffs to see if he washed out to sea but didn't see anything but raging surf. They searched for 3 days and never found the body.

Not quite in Hana but 5-6 miles past Honolua Bay right where the road turns to one lane. Totally desolate area for the most part. Took awhile for the first cop to show up. We talked with the search and rescue guys the next morning and they basically said they doubted they would find him. They wouldn't send anyone into the hole so were just hoping he would come out some how.

Don't fuck with Ma Nature.

Honorable mention at best for the coke taste tester.

I watched an episode of "The Indestructibles" this weekend that showed a guy that got knocked by a wave, and pushed into some cave. Made it out alive, which doesn't happen often.
