anyone fixed a large rivot that holds cuff to boot, inside one popped through shell creating mega flex in all directions.

I did a similar repair using a t-nut, I had to drill out a little bit of plastic but the repair has been solid. I don't know if that would work in your situation????

popped rivets on both adrenalines...garmont sent me replacement threaded models --no problems since...

Inside or outside edge?
It's a pretty easy fix for a good shop that has the correct tools.
I have a rivet press made for just this, I need to check to see if I have said rivets though.

Chainring bolts and washers have been known to work, but usually for a broken rivet, not a blown shell.

I've replaced ankle rivets with snowbored hardware.
Two base plate washers, a base plate bolt, and a nylock nut.

I've blown rivets in a pair of old touring boots. If the rivet doesn't fit the hole properly I think it does a ton more damage to shell by warping the hole and making it larger. I think the issue with nuts etc. is that with constant flexing it will back the bolt out and then you lose it while skiing. I've just taken it to the boot fitter and they pop some new rivets in there, or they have given me the canting adjust nut and washer that has a flared end to the bolt so if it does back out it won't back out all the way. They've never charged me for this piece of hardware.
Not sure how old a boot but that seems odd. A LOT of wear or bad plastic. T nuts as mentioned but as big a one that will work to make sure it doesn't pull through again. Washers may work but not if they push out to hammer your ankle. As said you likely will have to drill out the hole for a fit or at least to get is symmetrical again.
